News Summary

Allegiance Health Management in Louisiana has been hit with tax liens for over $17.7 million due to unpaid payroll taxes related to its Acadian Medical Center. The IRS placed a lien on the company’s property after multiple demands for payment were ignored. With reports indicating similar issues across six of its eleven clinics, the company’s ability to provide healthcare services may be jeopardized. Allegiance is working with the IRS to resolve the outstanding debts while navigating financial challenges in the rural healthcare landscape.

Shreveport, Louisiana – The Internal Revenue Service (IRS) has levied tax liens against Allegiance Health Management, a medical company based in Acadiana, for failing to pay over $17.7 million in payroll taxes related to its Eunice clinic, Acadian Medical Center. The tax liabilities reported span from the years 2019 to 2024.

The IRS’s notice indicates that despite repeated demands for payment, Allegiance has not settled the outstanding debts. As a result, a lien has been placed on all property and rights associated with the company. Allegiance Health Management’s CEO, Rock Bordelon, has stated that the company is cooperating with the IRS to clarify the actual amount owed, with an expectation that a resolution will occur soon.

This is not an isolated issue for Allegiance Health Management. Reports indicate that six of its eleven clinics currently have active liens totaling more than $34 million across Louisiana. The implications of these liens extend beyond just financial obligations; they may affect the company’s operations and its ability to provide healthcare services.

At the core of Allegiance’s operations is Acadian Medical Center, which serves as a full-service hospital and one of the largest employers in Eunice, employing over 300 people. The company has a stated mission to improve healthcare access in underserved rural areas, though rural hospitals have been facing significant financial challenges due to underfunding. In response, recent changes funded by the state of Louisiana, with federal approval, aim to address these funding issues.

Since acquiring Acadian Medical Center along with Mercy Regional Medical Center from LifePoint Health in 2018, Allegiance Health Management has expanded its reach in the region, managing additional facilities such as Northern Louisiana Medical Center (NLMC). However, NLMC itself is entangled in tax issues, with outstanding liabilities of around $7 million for unpaid payroll taxes that date back to March 2024. The most recent lien for NLMC was filed in August 2025, totaling $2.7 million.

Active liens against Allegiance-operated hospitals are reported in Caddo, Bossier, and Lincoln parishes, with many liens dating back to January 2023. Allegiance Health Management has a history of tax lien issues, including prior situations involving its hospitals in Arkansas. In 2018, the company reached a $1.7 million settlement to resolve a lawsuit concerning improper Medicare claims while denying any wrongdoing.

The company’s recent acquisitions, including a facility in Ruston in 2020 that has since closed, reflect its ongoing efforts to expand healthcare resources in the region. Furthermore, Bordelon’s entity, CLHG-Leesville, has an outstanding lien of approximately $20,000 dating back to November 2018. In November 2024, Bordelon also acquired Ruston’s Green Clinic with promises to enhance healthcare accessibility in the community.

As Allegiance Health Management navigates these challenges, it remains to be seen how the resolution of these tax liens will impact its operations and ability to fulfill its commitment to providing quality healthcare in Louisiana’s rural communities.
